( ) Select your classes each day in order of preference. Number legibly and please choose several classes per day. (You are responsible for paying for all classes to which you are assigned)

( ) Make sure the classes you have selected reflect your skill level.

( ) Include your self-addressed, stamped envelope for class assignments.

( ) Do not include class fees yet.

( ) Sign, date, and include Waiver with your registration form

descriptions; in some cases, the teacher will contact you beforehand to tell you
